Your OB Intake Appointment
Once you have had your ultrasound visit, your next visit in our office will be for an OB Intake Visit with our Perinatal Nurse Educator (PNE) and is scheduled between 8 to 10 weeks gestation.
During your visit, the PNE will discuss in great detail the medical, social and personal history of you and of the father of the baby. Some of these questions may be sensitive in nature but are necessary to fully determine your medical needs. Please keep that in mind when deciding who to bring with you to this appointment. Be prepared to answer these questions with this person(s) present. All information is kept in your permanent medical record and your confidentiality is fully protected. You will also be able to ask any questions you may have about your pregnancy and she can discuss any urgent concerns, if necessary, with the providers in the office. You will not see a provider at this visit.
In addition to talking with the PNE, you will have lab work drawn at this visit that will include screening for communicable diseases (HIV, Hepatitis, Syphilis, Rubella), checking your blood type, blood count, and a urine sample. Additional tests may be added based on your medical history. She will also discuss other optional genetic screening recommendations that are available at specific times during your pregnancy.
Our office actively coordinates care with our local health department and their Care Management for High Risk Pregnancy (CMHRP) program. As part of this collaboration, our office receives additional support to increase access to care and improve outcomes for our pregnant Medicaid population. The primary focus of this program is on preterm birth prevention. At your first visit with the PNE, all Medicaid patients will complete a screening questionnaire that will help us determine if you qualify for the program.
Please complete these documents prior to arriving at your first appointment:
